An online empowerment intervention for diabetes educators in self-management of diabetes: the guided self-determination program

For young adults with type 1 diabetes (YAWD) aged 18 to 35, it can be difficult to manage diabetes due to competing lifestyles and work or study-related commitments. Current educational interventions to achieve glycaemic control are not successful among those who have lost motivation for self-management. The Guided Self-determination (GSD) method is an evidence-based self-management program designed to guide both YAWD and diabetes educators through mutual reflection drawing on a number of semi-structured reflection sheets. The aim was to develop an online interactive learning package for Australian diabetes educators working with YAWD, based on GSD.

Nine experienced Australian diabetes educators trialled an online version of GSD. Based on their experiences and feedback, an e-learning package for educators was developed which includes theoretical teaching, self-study, individual and group supervision, assessments, and exercises with clients with diabetes. There is a high degree of interaction with diabetes educators' practical knowledge and experience to allow them to develop knowledge, skills and competencies in facilitating conversations with YAWD who have lost motivation. The e-learning package is based on Brinkerhoff learning principles.

The learning environment provides flexibility, by allowing both face-to-face and virtual learning. Participants can choose where and when to access the e-learning section. Differentiated learning is enabled as the material can be accessed from a digital platform where participants can repeat as required.

The online interactive GSD learning package was found to be a suitable, convenient and feasible way to engage educators. The participating diabetes educators suggested that the online communication was expedient, flexible and suitable for YAWD who have lost motivation. The novel GSD online e-learning package will improve diabetes education and care in Australia in two pivotal ways: by delivering truly individualised person-centred care and by facilitating 24/7 access to healthcare.
