AIMS: To evaluate the clinical and cost impacts of the All New Zealand Acute Coronary Syndrome Quality Improvement programme (ANZACS-QI) specifically for Māori with acute coronary syndrome (ACS). METHODS: Decision analytic Markov models were used to estimate the effectiveness and costs of the ANZACS-QI programme over four years of full coverage (2013 to 2016), against a hypothetical scenario in which the registry did not exist. The estimated return on investment (ROI) and incremental cost-effectiveness ratios (ICERs) are reported. RESULTS: The ROI ratio for the ANZACS-QI programme for Māori over the four-year period of full coverage was 1.51; that is, every dollar spent on the programme resulted in a return of NZD $1.51. The estimated ICER was NZD $114,786 per year of life saved (YoLS) over a one-year time horizon, but extending the benefits accrued to five years reduced the ICER to NZD $20,173 per YoLS. CONCLUSIONS: The ANZACS-QI programme represents a sound investment for improving outcomes in the setting of ACS for Māori in New Zealand. Using highly conservative assumptions, the programme would be cost-saving based on an annual ROI ratio of 1.5.
