Communicating about managing medications by older patients across transitions of care

This thesis explored communication processes between older patients and health professionals about managing medications across transitions of care. This research provided new knowledge about characteristics of medication communication between older patients and health professionals, sociocultural, environmental and organisational challenges affecting older patients' involvement in medication decisions and strategies enabling patient-centred medication communication in the context of care transitions.
